It takes up a significant amount of time filling this out during your appointment time and this will reduce the amount of time spent on your mental health concern. THESE FORMS WILL AUTOMATICALLY BE SENT TO YOU AS SOON AS YOU SIGN UP FOR YOUR APPOINTMENT VIA EMAIL AND TEXT MESSAGE.

Please be aware if you book an appointment and do not show or cancel within 24 hours of the appointment, you will be assessed a $75 no-show fee.
