It is imperative you complete the pre-visit assessments that will automatically be sent to your email PRIOR TO YOUR FIRST VISIT. It takes up a significant amount of time filling this out during your appointment time and this will reduce the amount of time spent on your mental health concern. 

Before your first visit

To be seen at Greener Hill, every patient must:

• Keep a valid credit or debit card on file. You’ll get a secure link to add it after booking. (Not required for Medicaid members.)
• Sign our office policies, HIPAA forms and Controlled Substance Treatment Agreement.
• Agree to our use of a secure AI-assisted note-taking tool during visits.
• Complete your intake forms before your first visit. They’re sent by email and text as soon as you book.

Missed appointments

Cancellations less than 24 hours before your visit and no-shows are charged a $75 fee to the card on file. Medicaid members are not charged this fee. Two no-shows or late cancellations within 12 months will result in discharge from the practice.

Need to be seen sooner?

Ask to be added to our waitlist. Cancellations open up often, so most people get in quickly. Call 252-606-4655 (opens at 8 AM).
