Greener Hill Psychiatric, PA – Notice of Privacy Practices

Effective date: October 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your rights

When it comes to your health information, you have the right to:

  • Get a copy of your records. Ask to see or get an electronic or paper copy of your medical record. We’ll provide it, usually within 30 days. We may charge a reasonable, cost-based fee.
  • Ask us to correct your record. If you think information is wrong or incomplete, ask us in writing to correct it. We may say no, but we’ll tell you why in writing within 60 days.
  • Request confidential communications. Ask us to contact you a certain way (for example, a specific phone or email) or at a different address. We will say yes to all reasonable requests.
  • Ask us to limit what we use or share. You can ask us not to use or share certain information. We don’t have to agree if it would affect your care. If you pay for a service in full out of pocket, you can ask us not to share that information with your health insurer, and we will say yes unless the law requires us to share it.
  • Get a list of those we’ve shared information with. Ask for a list of the times we shared your information in the 6 years before your request, who we shared it with and why. It excludes disclosures for treatment, payment, operations and ones you asked us to make. One list a year is free.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. If you’ve given someone medical power of attorney or someone is your legal guardian, that person can exercise your rights. We will confirm their authority first.
  • File a complaint if you feel your rights were violated. Contact us using the information at the end of this notice, or the U.S. Department of Health and Human Services Office for Civil Rights: 200 Independence Avenue SW, Washington, DC 20201; 1-877-696-6775; www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.

Your choices

You can tell us whether we may:

  • Share information with family, close friends or others involved in your care or payment for your care
  • Share information in a disaster relief situation

If you can’t tell us your preference, for example if you’re unconscious, we may share information if we believe it’s in your best interest. We may also share information when needed to lessen a serious and imminent threat to health or safety.

We never share your information for these purposes unless you give us written permission:

  • Marketing
  • Sale of your information (we do not sell health information)
  • Most sharing of psychotherapy notes

How we use and share your information

  • Treatment. We use your information and share it with other professionals treating you, such as your primary care provider, therapist or pharmacy.
  • Payment. We use and share information to bill and get paid by health plans and others, including through our billing service.
  • Running our practice. We use information to run the practice, improve care and contact you when needed, including appointment reminders by text, email or phone.
  • Business associates. We share information with vendors who help us operate, such as our electronic health record, AI-assisted documentation, billing and secure messaging services. Each signs a business associate agreement and must protect your information.

We may also share your information, as the law allows or requires, to:

  • Help with public health and safety (for example, reporting suspected abuse or neglect, or preventing a serious threat to someone’s health or safety)
  • Comply with the law, including when HHS asks to confirm we follow federal privacy law
  • Respond to organ and tissue donation requests and work with a medical examiner or funeral director
  • Address workers’ compensation, law enforcement and other government requests, including health oversight agencies and special government functions such as military and national security
  • Respond to lawsuits and legal actions, such as a court order or subpoena
  • Report to the North Carolina Controlled Substances Reporting System, and check it before prescribing controlled medications, as state law requires

Substance use disorder records

If we receive substance use disorder treatment records protected by federal law (42 CFR Part 2), we will not use or share those records in any civil, criminal, administrative or legislative proceeding against you unless you give written consent or a court orders it after notice to you.

Minors

Patients ages 14–17 are generally seen with a parent or legal guardian, who may access the minor’s records. When North Carolina law lets a minor consent to their own care, we follow the law on who may see that information.

Our responsibilities

  • We are required by law to keep your protected health information private and secure.
  • We will tell you promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. You can change your mind at any time by telling us in writing.
